by the National Archives.
Payment does not guarantee records can be retrieved if the
required information is missing or inaccurate. Please confirm you have access
to the details below before submitting your request.
- Veteran’s complete name used during military service
- Service number (if unknown, you may enter “unknown”)
- Social Security number
- Branch of service
- Rank
- Dates of service (approximations acceptable)
- Date and place of birth (especially if service number is unknown)
- Date of passing, if the Veteran is deceased
If you do not have this information, your request may be delayed or unable to
be completed.
